___________________________________________My interest lies in the integrated care of older adults, with a particular focus on their transition from the emergency department (ED) to the community. I am passionate about improving care coordination and ensuring that older individuals receive the support they need to recover safely and maintain independence after an ED visit. This includes addressing gaps in communication, follow-up care, and social services to promote better outcomes and reduce unnecessary hospital readmissions.
